Bramble Hub and Teneo Consulting help better integration of South-East London’s health services

Background

NHS organisations and local councils have established Sustainability and Transformation Partnerships (STPs) to improve health and care in 44 areas across England. The South-East London STP (SEL) is run by NHS Southwark Clinical Commissioning Group and local health providers. They identified the patient and system benefits that may be achievable through the implementation of an Accountable Care System.

The Requirement

SEL looked for support to identify the benefits and opportunities of accountable care, explore the best approach to developing a system and plan its implementation. In July 2017 SEL used Crown Commercial Service’s ConsultancyONE Framework, RM1502 to issue an Invitation to Quote for the work.

Our Success

SEL appointed Bramble Hub and Credo Business Consulting LLP, now trading as Teneo Consulting, to work with the commissioners and providers in SEL to design and implement a set of Accountable Care Support Services to better integrate primary care, acute care, community care, social care, and mental health services.
